If you are eligible to request a meeting this must be done in writing. The Facilitator receives all requests and appoints a Moderator to each one.

To submit your form through the website please download and save it to your device. When you open the form on your device you should be able to edit and save it. You can then submit it below by attaching it as a file.
Legacy arrangements for this service have now been put in place. These arrangements were discussed with all affected stakeholders so that a woman, or her dependents, would always have the option of a meeting, regardless of how much time had passed. To help with that, it was agreed that the Department of Health would take over the Facilitator’s administrative functions so that any woman, or her dependents, could still apply for a meeting into the future. You can read more about the Facilitator’s role here.
This means that if you apply for a meeting today, it will be a member of staff of the Department who administers your application. Your meeting will still be chaired by an independent moderator and you can read more about our moderators here. The meetings will remain completely confidential and no member of staff in the Department will have access to anything that you say in a meeting.
Information for those not eligible
As eligibility has been set out in legislation, you can only apply to us if you fall under one of the eligibility categories.
If you are not eligible to apply to us, you may be able to speak with the National Screening Service who has put in place a personal cervical screening review.
To request a review, you must have:
had a cervical cancer diagnosis and
had a CervicalCheck screening test in the 10 years before your diagnosis
The Screening Services advise that a review may help you or your family if you:
are looking for answers or ways to understand how and when your cancer developed
want to ask questions about your screening journey
